No one knew why my dad was there, asked the same questions for 3 days

My dad was in the hospital over Thanksgiving and we got so agitated and more stressed by the time the fifth person walked in not knowing why my dad was even there, asking the same questions over and over. I finally went home and came back with all his medications listed on a piece of paper and handed to them and said, "read it." People kept coming and going even a priest when my dad is Jewish.

Value based care is the new standard for reimbursements, and that especially means medical care coordination = no one should be walking into my dad's room without first having been briefed from the primary doctor/nurses - having already read the $100M electronic medical record (EMR); and only asking clarifications, doing a thorough assessment, and instructing on the plan of care with outcomes, timelines, and any specific-necessary doctor who is to be involved.
